The FallFrac screen method identifies patients at combined risk for falling and fracture. The method blends validated screening methods to stratify risk and associated treatment methods based on screen findings.
The validated screening tools include The FRACTURE Index, Morse Fall Risk assessment and the Fall Risk Assessment Tool (FRAT). These tools are combined with brief dizziness assessment and analyzed according to risk stratification criteria. A fall prevention pathway that is designed to work with this program is available upon request.
Data is quickly entered to the web-based application and fall prevention methods are recommended.

The FallFrac screen method is designed to work with the Bone Safety Evaluation to address musculoskeletal impairments that raise fall and fracture risk. The FallFrac screen method will enable a hospital to transition patients at risk to fall or fracture to an outpatient rehabilitation program where outcomes are tracked from initial inpatient admission to outpatient discharge.
